What is case management?
Case managers are responsible for integrating every part of a teen’s treatment—from therapy to academic assistance and family involvement—into a cohesive plan. Their efforts ensure all components work together to support your teen’s recovery.
To coordinate services, our case managers at Avery’s House collaborate with the rest of the team, including family members, teachers, medical doctors, and psychiatric providers, to keep everyone on the same page. However, case managers do more than coordinate care. They also fiercely advocate for their clients, working tirelessly to get them all necessary services and support.
Case management: A four-step process
- Assessment: We’ll start by doing a thorough assessment to help us understand your teen’s specific challenges, which may include trauma, social anxiety, or behavioral issues.
- Planning: Your child’s case manager will collaborate with their therapists to develop an individualized treatment plan that clearly outlines what your child needs to work on—for example, reducing anxiety or depression symptoms, improving family relationships, or learning coping techniques to manage stress. This process will also identify the evidence-based approaches most appropriate for your child.
- Implementation: The case manager will work closely with your teen, family members, and all support staff to ensure this treatment plan is carried out. This includes scheduling sessions with our licensed therapists, communicating with teachers to ensure necessary academic supports are in place, and connecting your teen to resources in their local community.
- Monitoring and evaluation: Your case manager will meet regularly with your child to monitor progress and tweak the treatment plan as needed. They’ll ask for feedback from your teen, family members, and other team members to guide these adjustments.

Our case managers aim for each teen in our care to be happy, healthy, and successful. This requires overseeing all aspects of treatment to help each teen function better in daily life, improve family and social relationships, develop resilience and healthy habits, and rediscover joy. To achieve these outcomes, our case managers
- Coordinate daily schedules, including weekly therapy and medical appointments, so that each teen gets comprehensive support.
- Communicate regularly with parents and other family, providing regular progress updates, involving them in treatment decisions, and offering guidance on how they can support their loved one throughout recovery.
- Work closely with our experienced treatment teams to identify and tackle any issues or concerns that arise during your teen’s journey.
- Manage discharge plans and ensure that each teen connects with community support services after they leave our program.
